In Compliance with the Michigan Board of Health Requirements; All Visits include the following:

• Review of medical tests, records and history.
• Consultations on pros and cons, alternative options, potential adverse side effects, and expected response to treatment.
• Plan of care stating the goals of therapy.
• Monitoring response to treatment.
• Communication with the patient’s primary care physician (only if authorizedby patient).
